When dealing with a traditionally Asian image with a bit of modern flare and design you can never go wrong with the traditional black & gray wash swirl with a few cherry blossoms thrown in. Can be made to work for about anywhere on your body. Go check out some traditional oriental tattoo designs.

Depending on how big you want to go, you could do anything with that, it's a neat design. You could add a dirt road under their feet and put a mountain scene behind it, as though they were coming out of the mountains. You could also make it look they are on the Great Wall, with some basic mountain scenes on the sides and behind it. Not sure where they are supposed to be, but, you could also make it look like he is coming out of a mine, with maybe a cave opening behind them. Many, many possibilities, what state are you in? I wouldn't mind doing one this one!!!

Depends where you're getting it, but personally I'd just add some shadows to make it look as if your skin is the surface they're standing on, as opposed to just being stuck on like a sticker. But with that design I would probably avoid adding much else in case it would detract from the detail of that piece.
